Completing the Survey On-line

This is a voluntary survey. You can choose to answer all of the questions or you can skip questions
you do not wish to answer. If you do not wish to complete the survey or prefer to complete the
survey on paper or in another language or require assistance, please advise your teacher.
Please respect the confidentiality and privacy of others.

1. Please wait for the teacher¡¦s instructions.
2. Turn on your computer. Notify your teacher if at any time your computer does not work.
3. Sign in with your usual computer User ID and Password. Then hit <- Enter. Click on the green
GO button.
4. Click on the yellow Here button for the OCDSB external website. This will take you to the OCDSB
home page.
5. Look under Upcoming Events (left side of homepage) and click on 2010-2011 OCDSB Student
Survey.
6. You are now on the Student Survey Information page.
7. To begin the survey, click on the button that says 2010-2011 OCDSB Student Survey. This will
take you to the Login Page.

8. To log into the survey, complete Steps A, B, and C (see sample above):
Step A) In the box Login ID, type your unique survey number from your sticker (e.g.: (TX0001))
Step B) Repeat this in the box that says Confirm ID.
Step C) Click on the Log In button. The Introduction Page will now come up on your screen.
9. Please read this page and when finished click the Next Page button.

10. This will bring you to the first page of your confidential survey.
11. Please read each question carefully and then click on your selected answer, for example:



12. In the example above, if your answer was “Never” then you would click this button.
13. Continue and read each question carefully. When you are ready to answer, click on the button that best reflects your answer.
14. The questions are voluntary. You may skip any question or change your answer at any time before you hit the submit button at the end of the survey.
15. At the bottom of each page, you are given two choices. Select one:

16. When you have completed the last question on the survey, you will see the following:

17. After you have pressed “Submit Survey”, you will receive a message stating:
Survey Successfully Sent
18. Close your browser.
19. Turn off your computer.
20. Hand your Student Instruction sheet back to the teacher as requested.
21. Please wait quietly while the other students complete the survey.
Note: Once your survey is submitted, your survey number cannot be re-used or accessed.
If others are still completing the survey, please respect their privacy.
