2010 - 2011 OCDSB Student Survey

INSTRUCTIONS FOR PARENTS AND GUARDIANS OF
STUDENTS IN JUNIOR KINDERGARTEN TO GRADE 6

We are pleased to provide information to help you complete the confidential 2010-2011 OCDSB Student Survey with your son or daughter. Completion of the survey is voluntary.  Parents and students can decide whether or not to complete the survey and can also skip question(s) they do not want to answer.

Please review these instructions carefully.

BEFORE YOU COMPLETE THE SURVEY

1. Determine whether you wish to complete the survey in paper or on-line.  If you choose to complete the survey on-line, please follow the instructions on the other side of this page.
2. If you require a translation of the survey or require assistance completing the survey including use of alternate format(s), please contact your school principal as soon as possible so that he or she can make appropriate arrangements.

Note: The survey is available in 7 additional languages; Arabic, Chinese, Farsi, French, Somali, Spanish and Urdu.  You may view a translated version of the survey on the OCDSB website at (http://www.ocdsb.ca/).  The on-line translations of the survey are for reference purposes only and cannot be completed. Copies for completion are available from your school principal.

COMPLETING THE PAPER SURVEY

1. Please set aside 20-30 minutes with your child to complete the survey. If you have more than one child in JK to grade 6, please complete one survey per child.
2. If you are completing the survey in another language, please transfer the confidential survey number from the top of page 1 on the English copy onto the translated copy.
3. Use a black or blue ink pen; avoid felt tip pens or marker.
4. Read each question carefully; some questions allow multiple answers.
5. Mark your answer(s) inside the response box, as indicated in the attached diagram.
6. When you have completed the survey, please review to ensure that you have considered every question; please also remember that the survey is printed on both sides of the page.
7. Remove the cover page from the survey and discard; verify that you have not signed the survey or written your child’s name on the survey.
8. Place the survey in the return envelope provided and seal the envelope. If you completed the survey with the assistance of a translated copy, please return both surveys in the envelope.
9. Return the sealed envelope to the school by May 20, 2011.

COMPLETING THE SURVEY ON-LINE

1. Detailed instructions to complete the survey on-line are on the reverse side of this page.
2. You will need your paper copy of the survey to transfer the survey number to log-on to the on-line survey.
3. After the survey is completed on-line, the paper survey can be discarded.

COMPLETING THE SURVEY ON-LINE

1.       On your home computer, go to the OCDSB website at http://www.ocdsb.ca/

2. Look under "Upcoming Events" and click on "2010-2011 OCDSB Student Survey".
3. On the Student Survey Information page, click on "2010-2011 OCDSB Student Survey".
4. This will take you to the Login Page. Follow Steps 1, 2, and 3 as seen below:

5. This will take you to the 2010-2011 OCDSB Student Survey Introduction Page.
6. Please read the Introduction Page and then click on the Next Page box.
7. This will bring you to the first page of your confidential 2010-2011 OCDSB Student Survey.
8. Please read each question carefully and then click on your answer, for example:

9. In the example above, if your answer was “Never” you would click this button.
10. Continue and answer each question by clicking on the button that best reflects your answer.
11. You may skip any question or change your answer(s) any time up until the time you press submit.  The questions are voluntary.
12. To deselect an answer, you may click your selection a second time.
13. If you exit the survey and did not press submit, you can re-enter with your password and the system will take you back to where you were in the process.
14. At the bottom of each page, you are given two choices. Select one:

Once you have pressed “Submit Survey”, you will receive a message stating: Survey Successfully Sent

Note: When your survey is submitted, your survey number cannot be re-used or accessed.
