Voluntary COVID-19 Reporting Dashboard

To support school communities with information about confirmed positive student or staff COVID-19 cases, the OCDSB has created a Voluntary COVID-19 Reporting Dashboard.

How does it work?

If a student or staff member has received a positive COVID-19 result from a rapid antigen test (RAT) or PCR test, you can complete the voluntary reporting form below

Students: https://forms.ocdsb.ca/Forms/COVID-Case-Collection-Student

It will ask to provide: student name, school, date of birth, date of symptom onset, and test type.

This information will then be matched with our internal records and anonymously posted to our database.

Parents/Caregivers may only submit for their children and students can only report for themselves. OCDSB staff will not submit information on behalf of students.

shboard provides information about cases voluntarily submitted by parents or students. It also includes information self-reported from staff members. 


The dashboard will include school, grade level and specific class for elementary students. For secondary students, the dashboard will provide school and grade level information only, it will not include individual course codes.

 

The Dashboard will include information about active and resolved cases. Cases are maintained on the active list on the dashboard for 5 days from symptom onset. After that, they move to resolved.


The information reported to the dashboard is voluntary and may not represent the full extent of COVID-19 cases in the OCDSB.
