To support school communities with information about confirmed positive student or staff COVID-19 cases, the OCDSB has created a Voluntary COVID-19 Reporting Dashboard.
How does it work?
If a student or staff member has received a positive COVID-19 result from a rapid antigen test (RAT) or PCR test, you can complete the voluntary reporting forms below:
Students: https://forms.ocdsb.ca/Forms/COVID-Case-Collection-Student
It will ask to provide: student name, school, date of birth, date of symptom onset, and test type.
This information will then be matched with our internal records and anonymously posted to our database.
Parents/Caregivers may only submit for their children and students can only report for themselves. OCDSB staff will not submit information on behalf of students.
